FirstBiotic Advanced contains 15 active ingredients: a prebiotic blend, a probiotic blend with multiple bacterial strains (including Streptococcus thermophilus, Lactobacillus acidophilus in two formulations, Bifidobacterium longum, Lb. bulgaricus, Lb. plantarum, Bif. infantis, Lb. casei, Lb. salivarius, Lb. rhamnosus, Bif. bifidum, Lb. paracasei, Lb. sporogenes, and Lb. reuteri), plus fructooligosaccharides—a type of prebiotic fiber that feeds beneficial bacteria. The capsule also contains inactive ingredients like glycerin, microcrystalline cellulose, and vegetable cellulose as binders and fillers.
Does it work?
Moderate evidence
The evidence for most conditions is incomplete. Lb. acidophilus and Bif. bifidum show possibly effective ratings for irritable bowel syndrome, and Lb. acidophilus is possibly effective for Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Lb. sporogenes is possibly effective for constipation and IBS. For the rest of the conditions in the data we hold—allergies, asthma, cognitive decline, diabetes, and others—the evidence is insufficient to rate these ingredients.
We don't have effectiveness information for Streptococcus thermophilus, fructooligosaccharides, or several of the Lactobacillus and Bifidobacterium strains on file.
How safe is it?
Well-documented data
Most of these probiotics are generally well tolerated in healthy people. However, anyone with a weakened immune system should check with their doctor before starting, because there's a small risk that live probiotic organisms could cause serious infection (bacteremia or sepsis) in that population, especially those with central lines or who are critically ill.
Fructooligosaccharides can cause gas, bloating, and abdominal pain, particularly at higher doses—clinical trials suggest doses under 10 grams daily are less likely to cause trouble. Uncommon gastrointestinal side effects from the probiotic strains themselves include abdominal discomfort, flatulence, and diarrhea.
During pregnancy, most of these strains are rated possibly safe, though food sources are preferred and supplement use should be discussed with your doctor first; safety data during breastfeeding is limited for several ingredients.

Formulation
Multi-Step Delivery System Phase 1 - Prebiotic Release A liquid prebiotic is released, providing nourishment for the probiotic bacteria to flourish and grow once it enters the intestine. It also supports a healthy gut pH and discourages further growth of unfriendly bacteria. Phase 2 - Gastrointestinal Protection FirstBiotic's inner capsule is protected from degradation, allowing it to travel past the stomach into the intestine. Phase 3 - Probiotic Multiplication The FirstBiotic probiotic blend is released into the intestine. The environment created by the prebiotic allows the probiotic bacteria to multiply and grow, maintaining a healthy environment in the gut.

Probiotic
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
