Nexabiotic contains 24 active ingredients: ten species and strains of Lactobacillus bacteria, five species of Bifidobacterium bacteria, two species of Bacillus bacteria, one yeast (Saccharomyces boulardii), one bacterial species (Streptococcus thermophilus), plus inulin, a prebiotic fiber. Together they're meant to support digestive and immune health.
The capsules also contain inactive ingredients — hydroxypropyl methylcellulose, pectin, water, rice dextrin, and medium chain triglyceride — which are fillers, binders, and carrier materials.
Does it work?
Moderate evidence
The evidence for individual ingredients varies widely. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum shows possible effectiveness for IBS and respiratory tract infections. Saccharomyces boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, Clostridioides difficile infection, and Helicobacter pylori.
Bacillus subtilis DE-111 is possibly effective for antibiotic-associated diarrhea, and inulin for obesity, constipation, and diabetes. For many other uses listed on the package — such as allergic rhinitis, anxiety, cognitive function, and asthma — the data we hold show insufficient reliable evidence.
Streptococcus thermophilus, for example, has not been established as effective for any of the six conditions listed for it in our data.
How safe is it?
Well-documented data
Most of these probiotics are generally well tolerated in healthy adults, but there are important cautions. Live-organism probiotics can, in rare cases, cause serious infections like bacteremia or sepsis in patients who are critically ill, immunocompromised, have central venous catheters, or are premature infants.
Common, mild side effects include bloating, flatulence, abdominal discomfort, and diarrhea — especially at higher doses of inulin (over 30 grams). Rarely, anaphylaxis has been reported with foods high in inulin.
One clinical trial reported rash and itching, though it's unclear whether the probiotics or another ingredient caused it. For pregnancy, Lactobacillus acidophilus, Bifidobacterium bifidum, Streptococcus thermophilus, and Bifidobacterium longum infantis are rated possibly safe, though data are limited.
For Lactobacillus helveticus, Lactobacillus gasseri, Bifidobacterium breve, Bacillus subtilis DE-111, and Lactobacillus delbrueckii, we hold no specific pregnancy or lactation safety rating — talk with your doctor or pharmacist about whether this product is right for you during pregnancy or while breastfeeding.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium bifidum
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Bacillus coagulans
Antibiotic Drugs
Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.
Lactobacillus helveticus

Lactobacillus delbrueckii preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. delbrueckii preparations by at least two hours.
Inulin
Antidiabetes Drugs
Theoretically, inulin might increase the risk of hypoglycemia with antidiabetes drugs.
Some clinical research shows that inulin improves glycemic control in patients with diabetes; however, it is unclear if it has hypoglycemic effects.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
