Agmatine and L-Citrulline are both generally well tolerated short-term, but long-term safety in humans isn't well studied. Common side effects from agmatine include diarrhea, dyspepsia (indigestion), and nausea — these appeared in a small number of trial participants within 2-3 days and resolved when they stopped.

L-Arginine commonly causes abdominal pain, bloating, nausea, diarrhea, headache, and flushing. L-Citrulline most often causes gastrointestinal discomfort and heartburn.

Agmatine is not recommended during pregnancy or while breastfeeding due to insufficient safety data. L-Citrulline should also be avoided in pregnancy and breastfeeding.

Theoretically, concomitant use of L-arginine and ACE inhibitors may increase the risk for hypotension and hyperkalemia.
Combining L-arginine with some antihypertensive drugs, especially ACE inhibitors, seems to have additive vasodilating and blood pressure-lowering effects. Furthermore, ACE inhibitors can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ACE inhibitors with L-arginine may increases the risk of hyperkalemia.
