Both nattokinase and serrapeptase are generally well tolerated in the short term, but long-term safety data — especially for serrapeptase — is limited. The main concern is their blood-thinning effects: rare cases of bleeding have been reported with nattokinase, including the serious case of brain hemorrhage mentioned above.

Serrapeptase users have occasionally reported stomach pain, upset stomach, and nausea, though these occurred at the same rate as placebo in trials. Nattokinase can trigger allergic reactions in people sensitive to natto; symptoms of natto allergy typically appear within two hours and affect the throat and voice box.

Pregnancy and breastfeeding: the safety data advises against using nattokinase during pregnancy and while breastfeeding. The same applies to serrapeptase — not enough safety information exists for either stage, so both should be avoided.

Suggested use: adults take one (1) capsule three times a day, 30 to 60 minutes before, or two hours after a meal. Take with an 8oz. glass of water. Take for 2 weeks, then reduce dosage to one (1) capsule twice daily for 2 weeks.

Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
