BIOHM Total Probiotic is a 5-ingredient capsule built around live beneficial bacteria and yeast. The active ingredients are L. acidophilus, L. rhamnosus, S. boulardii (a beneficial yeast), B. breve, and amylase (an enzyme).
These are meant to support digestive health. The capsule itself contains cellulose and hypromellose as inactive ingredients.
Does it work?
Moderate evidence
The evidence for these ingredients is mixed. L. acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis, but there isn't enough reliable data yet on its use for acute respiratory distress syndrome.
S. boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, Clostridioides difficile infection, and Helicobacter pylori. B. breve shows insufficient evidence for allergic rhinitis and antibiotic-associated diarrhea, and appears possibly ineffective for age-related cognitive decline and sepsis.

How safe is it?
Well-documented data
L. acidophilus is generally well tolerated in healthy adults and children, though rare cases of serious infection (such as bacteremia or endocarditis) have been reported — most likely in people whose bacteria translocated from elsewhere in the body. Mild GI side effects like bloating, flatulence, and abdominal discomfort can occur but are uncommon.
S. boulardii is generally safe for healthy adults but should be avoided if you're critically ill or immunocompromised; very rarely it has caused fungemia, especially in ICU patients with central lines. Mild GI complaints like nausea and flatulence are rare.
B. breve is generally well tolerated in healthy people but caution is advised if you're severely ill or immunocompromised; rare cases of bacteremia have occurred in preterm infants and critically ill patients. Bloating and flatulence are possible.
All three live-organism ingredients carry a small risk of serious infection in vulnerable populations — talk with your doctor before use, especially if you're critically ill, have a central line, or a weakened immune system.

L. acidophilus 16 axg
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
B. breve 19 bx
Antibiotic Drugs
Theoretically, taking Bifidobacterium breve with antibiotic drugs might decrease the effectiveness of B. breve.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.
S. boulardii 16 mxg
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
