Tebentafusp-tebn falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with pairing American Hellebore and Kimmtrak (tebentafusp). The alkaloids in this plant can stretch out the QT interval, which is the electrical timing of your heartbeat. Tebentafusp can do the same thing during treatment. When you stack two QT-stretching effects together, you raise the odds of a dangerous rhythm called torsades de pointes, which can cause fainting, a racing or irregular heartbeat, or in rare cases a cardiac emergency.

This isn't a metabolism issue, and tebentafusp isn't a prodrug, so the worry is simply two heart-rhythm effects adding up. American Hellebore is also genuinely poisonous, so I'd steer clear of it entirely. Please loop in your oncology team or pharmacist before taking any product that contains it.

What Is Tebentafusp injection Used For?

Brand name: Kimmtrak

Kimmtrak (tebentafusp) is used to treat adults with unresectable or metastatic uveal melanoma — a type of melanoma that starts in the eye and cannot be removed by surgery or has spread to other parts of the body. It is only for patients who test positive for a specific genetic marker called HLA-A*02:01.

Before starting Kimmtrak, a blood test is required to confirm that you carry the HLA-A*02:01 marker. Only patients who test positive are candidates for this treatment.
