Here's the gist with alogliptin (Nesina): this avocado sugar extract has been studied mostly in test tubes and animals, where its main compound (D-mannoheptulose) may nudge blood sugar upward by interfering with how cells use glucose. If that happens in people, it could work against what alogliptin is trying to do, which is lower your blood sugar. The worry is weaker blood sugar control, meaning numbers that run higher than expected.

I want to be honest, though: this is a theoretical concern based on lab data, not solid human studies, so I wouldn't lose sleep over it. Don't stop or change your alogliptin. Just give your pharmacist or doctor a heads-up before adding this supplement, and keep an eye on your readings.
