Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the basic idea with acalabrutinib (Calquence). Your body relies on an enzyme called CYP3A4 to clear this cancer drug out of your system. The beta-carboline compounds in ayahuasca (the same ones that act as MAOIs) may slow that enzyme down. If CYP3A4 is partly blocked, acalabrutinib could build up to higher levels than expected, which can mean more side effects like bruising or bleeding, headaches, infections, or low blood counts.

I want to be honest about the strength here: this is theoretical, based only on lab studies, and it hasn't been shown in actual people. Even so, with a cancer medicine you don't want surprises. Please loop in your oncology team or pharmacist before mixing these, especially since ayahuasca carries its own serious risks.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
