Why this applies to Brexpiprazole

Brexpiprazole falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the concern with mixing ayahuasca and Rexulti (brexpiprazole). Ayahuasca contains compounds that act like MAO inhibitors and also boost serotonin activity in the brain. Brexpiprazole isn't a classic serotonin booster, but it does touch serotonin pathways, so stacking it with ayahuasca could theoretically push serotonin too high. That raises the risk of serotonin syndrome, which can show up as shivering, sweating, tremors, a racing heart, confusion, and nausea.

I want to be honest that this is mostly a theoretical, pharmacology-based caution rather than something proven for this exact drug. Still, ayahuasca is potent and unpredictable, so please don't combine the two without talking to your doctor or pharmacist first. Don't change your brexpiprazole on your own either.

Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the short version for Rexulti (brexpiprazole). Your body relies partly on an enzyme called CYP2D6 to clear this medicine out. Ayahuasca contains compounds (harmine and harmaline) that may block that enzyme. If CYP2D6 is slowed down, brexpiprazole can build up in your blood, which could mean more side effects than usual, things like drowsiness, dizziness, restlessness, weight gain, or feeling lightheaded when you stand up.

I want to be honest about the strength of this, though. So far this blocking effect has only been seen in lab studies, not in real people, so it's theoretical. On top of that, ayahuasca is a potent, illegal-in-many-places brew that can cause serious reactions on its own. Please don't combine them without talking to your doctor or pharmacist first.

Here's the gist for Rexulti (brexpiprazole). Your liver uses an enzyme called CYP3A4 to help clear this medicine out of your body. Compounds in ayahuasca (the beta-carboline alkaloids) may block that enzyme, at least in lab studies. If that happens in a real person, brexpiprazole could build up to higher levels than usual and lead to stronger side effects, like drowsiness, dizziness, restlessness, or low blood pressure when you stand up.

I want to be honest about the strength of this: it's theoretical, based on test-tube research, and hasn't actually been seen in people. Still, ayahuasca itself is risky and is also a strong MAOI, which is a separate concern with brain-active drugs. Please don't change anything on your own. Loop in your doctor or pharmacist first.

Here's the thing with mixing ayahuasca and brexpiprazole (Rexulti). Brexpiprazole is an antipsychotic that fine-tunes dopamine activity in your brain. Ayahuasca contains a compound called harmine, and in animal studies it seems to push dopamine levels up. On paper, those two could be tugging at the same system in opposite directions, so the worry is that ayahuasca might blunt how well your medication works or stir up agitation.

I want to be honest that this is theoretical. It's based on lab and animal research, not real patient cases, so we don't truly know how much it matters in people. That said, ayahuasca also acts as an MAOI and is a powerful, unpredictable brew, so please don't combine it without first talking to your doctor or pharmacist.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
