Why this applies to Busulfan

Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the basic idea with busulfan and ayahuasca. Your body relies on a liver enzyme called CYP3A4 to help clear busulfan. The beta-carboline compounds in ayahuasca (harmine and harmaline) have been shown in lab dish studies to slow that enzyme down. If that happened in people, busulfan could build up higher than intended, and with a chemo drug that matters a lot. More drug in your system can mean worse nausea, mouth sores, low blood counts, and a higher risk of serious side effects like liver and lung problems.

I want to be honest, though: this is only seen in test tubes, not in humans, so it's theoretical. That said, ayahuasca carries its own real dangers and isn't recommended. Please talk with your oncology team or pharmacist before mixing anything new with busulfan.

What Is Busulfan Used For?

Brand names: Myleran, Busulfex

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.

Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
