Why this applies to Clozapine

Clozapine falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the concern with mixing ayahuasca and clozapine. The vine in ayahuasca acts like an MAO inhibitor and, along with its DMT, pushes serotonin activity up in the brain. Clozapine also nudges serotonin signaling as part of how it works. Stacking the two could, in theory, lead to too much serotonin, which is called serotonin syndrome. Watch-for signs include shivering, sweating, tremors, a racing heart, agitation, confusion, nausea, or vomiting.

I want to be straight with you: this warning is theoretical, based on pharmacology and a single case report involving a different antidepressant, not clozapine itself. Still, clozapine is a serious medicine with its own risks, and ayahuasca can be dangerous on its own. Please loop in your doctor or pharmacist before combining anything.

Clozapine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist for clozapine: your body uses several liver enzymes to clear it, and one of them is CYP2D6. The beta-carboline compounds in ayahuasca (harmine and harmaline) may block that enzyme. If clearance slows, more clozapine can linger in your blood, which could mean stronger effects and more side effects like heavy drowsiness, dizziness, drooling, a racing heartbeat, low blood pressure, or constipation.

That said, this is mostly a theoretical concern from lab studies, not something proven in people, so don't panic. But clozapine is a tricky, closely monitored drug, and ayahuasca itself carries real risks. Please check with your doctor or pharmacist before mixing the two.

What Is Clozapine Used For?

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
