Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for your HIV pill, doravirine (Pifeltro). Your body relies on an enzyme called CYP3A4 to clear this medication out. Lab studies hint that the beta-carboline compounds in ayahuasca (harmine, harmaline) may slow that enzyme down. If that happened, doravirine could build up a bit higher than usual, which might mean more of its typical side effects like headache, nausea, dizziness, or odd dreams.

Now, the honest part: this is purely theoretical, based on test-tube work, and has never actually been shown in people. So I wouldn't lose sleep over it. The bigger concern is that ayahuasca itself can be risky and is illegal in many places. Please loop in your doctor or pharmacist before mixing anything new with your HIV regimen.

About Doravirine

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used to treat HIV-1 infection in adults and in children and adolescents who weigh at least 35 kg (about 77 lbs). It is always used in combination with other antiretroviral medicines — never on its own.

Pifeltro is approved for two groups: people who have never taken HIV treatment before, and people who are already virologically suppressed (meaning HIV is undetectable in their blood) on a stable regimen and need to switch, provided they have no history of treatment failure or known resistance to doravirine.
