Elexacaftor, Tezacaftor, Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea. All three pieces of Trikafta (elexacaftor, tezacaftor, and ivacaftor) are cleared from your body mainly by a liver enzyme called CYP3A4. Ayahuasca's vine compounds (the beta-carbolines) can slow that same enzyme down, at least in lab testing. If that happens in real life, your Trikafta could build up higher than expected, which might mean more side effects like headache, belly pain, diarrhea, rash, or dizziness.
I want to be honest about the strength here: this is theoretical, based only on test-tube work, and it hasn't been confirmed in people. Still, ayahuasca carries its own serious risks. Please don't change anything on your own. Check with your CF team or pharmacist first.

Trikafta (elexacaftor, tezacaftor, and ivacaftor) is used to treat cystic fibrosis (CF) in adults and children aged 2 years and older who have a clinical diagnosis of CF and carry at least one CFTR gene variant that is expected to respond to the medication — either based on clinical trial data or laboratory testing.
If a patient's genetic variant is not already known, an FDA-cleared CF genetic test should be used to confirm eligibility before starting treatment.
