Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Let me walk you through this one. Padcev (enfortumab vedotin) is an antibody-drug combination that delivers a chemo payload (called MMAE) into cancer cells, and that payload is partly cleared by a liver enzyme called CYP3A4. Lab studies suggest the beta-carboline compounds in ayahuasca can block CYP3A4. If that blocking happened in people, a bit more of the chemo portion could linger in your body, which in theory might add to side effects like numbness or tingling in the hands and feet, skin rash, fatigue, nausea, or higher blood sugar.

Keep this in perspective though. This concern comes only from test-tube research and has never been confirmed in humans. Even so, ayahuasca is risky on its own, so please talk with your oncologist or pharmacist before mixing anything new with Padcev.

What Is Enfortumab vedotin-ejfv Injection Used For?

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
