Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

This combo pill pairs ibuprofen (an anti-inflammatory) with hydrocodone, an opioid. Here's the wrinkle: hydrocodone is partly a prodrug, meaning your body uses the liver enzyme CYP2D6 to turn it into hydromorphone, a stronger painkiller. Ayahuasca's beta-carboline compounds (harmine and harmaline) may block CYP2D6.

If that enzyme is slowed, less hydrocodone gets converted, so you could actually get less pain relief than expected. On the flip side, these MAOI-type compounds carry their own serious risks with opioids, so this is not a casual mix. Honestly though, the CYP2D6 part is only from lab studies, not proven in people. Please talk with your pharmacist or doctor before combining these.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist with this combo pill (ibuprofen plus hydrocodone). The hydrocodone half relies on liver enzymes, including CYP3A4, to be processed. Lab studies hint that the beta-carboline compounds in ayahuasca can block CYP3A4, which in theory could let hydrocodone build up a bit and lead to stronger opioid effects like extra drowsiness, slowed breathing, constipation, or feeling foggy. I want to be honest, though: this is only seen in test-tube research, not in real people, so it's more of a caution than a proven problem.

There's a bigger reason to be careful here. Ayahuasca acts like an MAOI, and mixing MAOIs with opioids can rarely cause serotonin reactions or dangerous reactions on its own. Please check with your doctor or pharmacist before combining these.
