Ayahuasca and Ivacaftor, Lumacaftor

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Ivacaftor, Lumacaftor

Ivacaftor, Lumacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with Orkambi (lumacaftor/ivacaftor). The ivacaftor part is cleared from your body mainly by a liver enzyme called CYP3A4. Ayahuasca contains beta-carboline compounds that, in lab studies, can slow that same enzyme down. If that slowdown happened in a real person, ivacaftor could build up a bit higher than usual, which might mean more side effects like headache, dizziness, stomach upset, or changes in liver enzyme levels.

That said, this is theoretical. It comes only from test-tube research and has not been shown in actual people, so I don't want to overstate it. Ayahuasca also carries its own serious risks regardless of your medication. Please check with your pharmacist or doctor before mixing the two.

From our pharmacist’s overview on the Ivacaftor, Lumacaftor page

“Ivacaftor with lumacaftor (Orkambi) only works against cystic fibrosis if its blood level stays steady, and most of the supplements on this page can tug that level up or down through a shared liver pathway called CYP3A4. Grapefruit can push the level higher, St.”

About Lumacaftor and Ivacaftor

What Is Lumacaftor and Ivacaftor Used For?

Brand name: Orkambi

Lumacaftor and ivacaftor (Orkambi) treats cystic fibrosis (CF) in people aged 1 year and older who are homozygous for the F508del mutation in the CFTR gene. Homozygous means you inherited the mutation from both parents.

If your genetic makeup is unknown, an FDA-cleared CF mutation test is used to check for the mutation on both copies of the gene. Orkambi has not been shown to be safe or effective in people with CF who do not have two copies of F508del.
