Why this applies to Meperidine, Promethazine

Meperidine, Promethazine falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

This one deserves real caution. The combo pill Meprozine/Mepergan contains meperidine (a strong opioid pain reliever) plus promethazine. Meperidine isn't just any opioid, it also raises serotonin in the body. Ayahuasca contains MAO inhibitors (harmine and harmaline) plus DMT, which also push serotonin up. Stacking the two is one of the classic recipes for trouble.

Mixing meperidine with an MAOI can lead to serotonin syndrome, with shivering, sweating, agitation, fast heartbeat, muscle twitching, high fever, and confusion. This can come on fast and be dangerous. Even though our file rates the evidence as theoretical, the meperidine plus MAOI risk is taken very seriously in real practice. Please don't combine these without talking to your doctor or pharmacist first.

Meperidine, Promethazine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the situation with Meprozine or Mepergan, which pairs meperidine (an opioid pain reliever) with promethazine (an antihistamine for nausea and sedation). Lab studies suggest the beta-carboline compounds in ayahuasca (harmine and harmaline) can slow down the CYP2D6 enzyme, one of the pathways your body uses to clear both of these drugs. If that pathway is blocked, the medicine could build up and hit harder, meaning more drowsiness, dizziness, slowed breathing, low blood pressure, and confusion.

But there's a bigger concern here. Those same ayahuasca compounds act as MAOIs, and meperidine is famously dangerous with MAOIs, risking serotonin toxicity (high fever, agitation, rigid muscles, racing heart). This is a combination to avoid. Please talk with your doctor or pharmacist before mixing these.
