Ayahuasca and Rimegepant

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Rimegepant

Rimegepant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with Nurtec ODT (rimegepant). Your body clears this migraine medicine using a liver enzyme called CYP3A4. The beta-carboline compounds in ayahuasca may slow that enzyme down, at least in lab studies. If the enzyme works slower, rimegepant could hang around in your blood longer and at higher levels, which may mean a stronger effect and a higher chance of side effects like nausea or stomach upset.

I want to be honest about the strength of this, though. So far this has only been seen in test-tube research, not in actual people, so it's more of a theoretical concern than a proven problem. Setting that aside, ayahuasca carries its own serious risks. Please talk with your doctor or pharmacist before combining anything with it.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Rimegepant page

“Rimegepant (Nurtec ODT), the dissolving migraine tablet, calls for real care alongside supplements, and three entries deserve your attention before anything else. Grapefruit can block the CYP3A4 enzyme that clears rimegepant, raising its blood levels and the chance of side effects.”
