Siponimod falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the idea behind this pairing. Your body partly relies on a liver enzyme called CYP3A4 to clear siponimod from your system. Lab studies suggest the beta-carboline compounds in ayahuasca (harmine and harmaline) can block that same enzyme. If that blocking happened in people, siponimod could build up to higher levels than expected, which may mean more side effects like a slowed heart rate, dizziness, headache, higher blood pressure, or liver irritation.

I want to be honest about the strength of this, though. So far this is only seen in test-tube research, not in actual patients, so it's theoretical. That said, ayahuasca carries its own serious risks and isn't safe to mix casually. Please loop in your doctor or pharmacist before combining anything with your MS medication.

What Is Siponimod Used For?

Brand name: Mayzent

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).

It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
