Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Vosevi (sofosbuvir, velpatasvir, and voxilaprevir). The beta-carboline compounds in ayahuasca have been shown, in lab dish studies only, to slow down an enzyme called CYP3A4. Parts of Vosevi, especially the voxilaprevir piece, are handled by that same enzyme. In theory, blocking it could let more of the drug build up in your blood, which might mean a higher chance of side effects like headache, nausea, tiredness, or diarrhea.

I want to be honest though, this is only a theoretical concern from test-tube research and hasn't actually been seen in people. The bigger worry is that ayahuasca is a strong, risky psychoactive brew on its own. Please don't add anything to your hepatitis C treatment without checking with your doctor or pharmacist first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
