Why this applies to Trazodone

Trazodone falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the concern with mixing trazodone and ayahuasca. Trazodone raises serotonin in your brain to help with depression and sleep. Ayahuasca does something similar in two ways: its DMT acts on serotonin pathways, and the vine compounds (harmine and harmaline) work like MAO inhibitors, which slow how your body breaks down serotonin. Stack those together and serotonin can climb too high.

That can lead to serotonin syndrome, with symptoms like shaking, sweating, racing heart, agitation, fever, nausea, and confusion. This is mostly theoretical, based on lab and animal work plus case reports with similar drugs, so it isn't certain. But the risk is serious enough that I'd really encourage you to talk with your doctor or pharmacist before combining these.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist for trazodone. The beta-carbolines in ayahuasca (harmine and harmaline) may block an enzyme called CYP2D6, which is one of the routes your body uses to clear trazodone. If that pathway slows down, trazodone can build up a bit higher than usual, which might mean more drowsiness, dizziness, low blood pressure when you stand, or a groggy hangover feeling.

I want to be honest about the strength of this: it's theoretical, based on lab studies, and hasn't actually been shown in people. The bigger concern by far is that ayahuasca also acts as an MAOI, and combining MAOIs with serotonin drugs like trazodone can be genuinely dangerous. Please talk with your doctor or pharmacist before mixing these.

Here's the gist for trazodone: your liver uses an enzyme called CYP3A4 to help clear it from your body. The beta-carboline compounds in ayahuasca may slow that enzyme down. If they do, trazodone could build up a bit higher than usual, which might mean more of its typical effects like drowsiness, dizziness, low blood pressure, or grogginess.

I'll be honest, this particular concern only comes from lab studies, not real people, so it's theoretical and graded as weak evidence. The bigger worry with this combo is actually serotonin.
