Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Guaifenesin, Theophylline
Guaifenesin, Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Baikal Skullcap interacts with.
In vitro evidence suggests that constituents of Baikal skullcap inhibit the activity of CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product pairs guaifenesin (a simple expectorant that thins mucus) with theophylline, which opens up your airways. The part to watch here is the theophylline. Your liver clears it using an enzyme called CYP1A2, and lab studies suggest compounds in Baikal skullcap may slow that enzyme down. In theory, that could let theophylline build up in your blood.
That matters because theophylline has a narrow safety window. Too much can cause nausea, a racing or pounding heart, jitteriness, trouble sleeping, or in serious cases seizures. Important: this is only test-tube evidence and hasn't been seen in real people. Still, since theophylline is touchy, check with your pharmacist or doctor before combining them.
