Baikal Skullcap and Loncastuximab Tesirine-Lpyl

Here is the interaction we found between these two, with the evidence behind it.

Minor

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Loncastuximab Tesirine-Lpyl

Loncastuximab Tesirine-Lpyl falls under P-GLYCOPROTEIN SUBSTRATES — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase levels of drugs transported by P-glycoprotein.
In vitro and animal research suggests that baicalein, oroxylin A, and wogonin, constituents of Baikal skullcap, can inhibit P-glycoprotein. This effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Zynlonta (loncastuximab tesirine) and Baikal skullcap. This herb contains plant compounds (baicalein, wogonin, and others) that, in lab and animal studies only, can slow down a transporter called P-glycoprotein. Since this medication is considered a P-glycoprotein substrate, the theory is that skullcap could let a bit more of the drug build up in your body.

I want to be honest about how soft this is. This effect has never actually been shown in people, and it's rated as a minor, theoretical concern. If it did matter, you might see more of this drug's usual side effects, like low blood counts, fatigue, swelling, or rashes. Since this is a serious cancer infusion, please run any supplement past your oncology team before adding it.

“Loncastuximab tesirine (Zynlonta) is an infusion treatment for lymphoma that has come back or stopped responding, so I treat anything that can shift its levels as a real safety question.
