Pioglitazone falls under ANTIDIABETES DRUGS — the group Baikal Skullcap interacts with.

Theoretically, concomitant use of Baikal skullcap with antidiabetes drugs might enhance blood glucose lowering effects.
Baicalein, a constituent of Baikal skullcap, has alpha-glucosidase inhibitory activity in vitro. Animal research also suggests that Baikal skullcap enhances the antidiabetic effects of metformin. However, in a small human study, taking Baikal skullcap extract did not enhance the antidiabetic effects of metformin, although it did modestly lower glucose levels during an oral glucose tolerance test (OGTT). Until more is known, use cautiously.

Here's the gist if you take pioglitazone (Actos) and want to try Baikal skullcap. This herb may have a mild blood sugar lowering effect of its own. So when you stack it on top of your pioglitazone, the two could add up and push your numbers a little lower than expected. That raises the chance of low blood sugar (hypoglycemia), which can feel like shakiness, sweating, dizziness, or a sudden headache.

I want to be honest about the evidence: this is mostly theoretical, based on lab and animal work, with only weak human data. It's not a strong or certain interaction. Still, if you're checking your sugars and considering this herb, give your pharmacist or doctor a heads up first so they can watch your readings.

What Is Pioglitazone Used For?

Brand name: Actos

Pioglitazone tablets — sold as Actos and under the generic names Pioglitazone and Pioglitazone Hydrochloride — are used to improve blood sugar (glucose) control in adults with type 2 diabetes. They are taken along with diet and exercise, either alone or combined with other diabetes medicines such as a sulfonylurea, metformin, or insulin.

Pioglitazone is not for type 1 diabetes or diabetic ketoacidosis (a serious complication where the blood becomes too acidic). It only works when the body is still producing some of its own insulin.
