Nalidixic Acid falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Banaba interacts with.

Theoretically, concomitant use of banaba with substrates of OATP might reduce the bioavailability of the OATP substrate.
In vitro research shows that banaba inhibits OATP, particularly OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the absorption of drugs and other compounds.

Here's the gist with banaba and your antibiotic, nalidixic acid. Banaba contains compounds that, in lab dishes, seem to block a gut transporter called OATP (especially a version known as OATP2B1). This transporter helps shuttle certain drugs from your intestine into your body. If banaba slows that doorway down, less of the antibiotic might get absorbed, which could mean weaker levels in your bloodstream and a urinary tract infection that doesn't clear the way it should.

I want to be honest about how soft this evidence is. This comes only from test-tube studies, not from people, so we can't say it actually happens in real life. Still, since an antibiotic needs to reach a strong enough level to work, it's worth a quick chat with your pharmacist or doctor before pairing the two.
