Bay Leaf and Phenytoin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Phenytoin

Phenytoin falls under CNS DEPRESSANTS — the group Bay Leaf interacts with.

Theoretically, taking bay leaf in large amounts may enhance the therapeutic and adverse effects of sedatives.
Bay leaf contains methyl eugenol. Animal research shows that methyl eugenol has sedative properties. Avoid concomitant use.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Phenytoin (Dilantin). Bay leaf contains a compound called methyl eugenol, and in animal studies it seemed to have a mild sedating effect. The thinking is that if you take large, concentrated amounts of bay leaf, that calming effect could add to the drowsiness phenytoin can already cause, leaving you more tired, foggy, or unsteady on your feet.

I want to be honest that this is purely theoretical and based on animal research, not human studies. The pinch of bay leaf in your soup is not a concern at all. Phenytoin is also a tricky drug where blood levels matter a lot, so before starting any concentrated bay leaf supplement or essential oil, run it past your pharmacist or doctor first.

From our pharmacist’s overview on the Phenytoin page

“Phenytoin (Dilantin) is a seizure medicine with a narrow therapeutic index, meaning small changes in its blood level can bring seizures back or tip over into toxicity, so I treat its supplement list with real seriousness. The biggest group, 78 ingredients, piles on sedation, and alcohol, kava, and GHB all rate major for exactly that re…”
