Here's the gist for trimethoprim (Proloprim, Trimpex) and bitter orange. Trimethoprim is one of the antibiotics that can, in some people, slightly affect the heart's electrical timing (what doctors call the QT interval). Bitter orange contains synephrine, a stimulant that can nudge up your heart rate and blood pressure. The concern is that the two together might add up and, in rare cases, throw off the heart's rhythm.

I want to be straight with you: this is mostly theoretical, based on a single case report involving bitter orange plus caffeine, so it's weak evidence and unlikely for most healthy folks taking a short antibiotic course. Still, if you have a heart condition, high blood pressure, or take other stimulants, please check with your pharmacist or doctor before combining them.

What Is Trimethoprim Used For?

Brand names: Proloprim, Trimpex

Trimethoprim tablets are used to treat initial episodes of uncomplicated urinary tract infections (UTIs). It works against specific bacteria — including E. coli, Proteus mirabilis, Klebsiella pneumoniae, Enterobacter species, and coagulase-negative Staphylococcus species (including S. saprophyticus).

It should only be used when there is a proven or strongly suspected bacterial infection. Your doctor may order a urine culture to confirm which bacteria are present and whether trimethoprim will work against them.
