Black Tea and Ephedrine Sulfate, Hydroxyzine, Theophylline

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Black Tea interacts with whole categories of medication. Ephedrine Sulfate, Hydroxyzine, Theophylline falls into 2 of those categories, so it can interact with Black Tea in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

STIMULANT DRUGS

1 of 2

Why this applies to Ephedrine Sulfate, Hydroxyzine, Theophylline

Ephedrine Sulfate, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Black Tea interacts with.

Theoretically, concomitant use might increase stimulant adverse effects.
Black tea contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product contains stimulants, especially ephedrine and theophylline, which already rev up your nervous system. Black tea adds its own caffeine on top of that. The concern here isn't that the tea changes how your body breaks the medicine down. It's simply that two stimulants stacked together can push their effects further than either alone.

In real life, that might look like a faster or pounding heartbeat, jitteriness, trouble sleeping, anxiousness, or a temporary bump in blood pressure. Theophylline is a narrow-margin drug, so even modest jitter or a racing pulse is worth paying attention to. This caution is based on expert opinion rather than hard studies, so an occasional cup likely won't be dramatic. Still, if you notice your heart racing or feel wired, check in with your pharmacist or doctor.

THEOPHYLLINE

Ephedrine Sulfate, Hydroxyzine, Theophylline also falls under THEOPHYLLINE.

Theoretically, black tea might increase the levels and adverse effects of theophylline.
Black tea contains caffeine, which can increase theophylline levels.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the thing to keep an eye on with this combination: your medicine contains theophylline, and the caffeine in black tea is chemically very similar. Both are processed by your liver in much the same way, so they can compete and end up pushing theophylline levels a little higher than expected. This isn't a prodrug situation, so the worry is too much effect, not too little.

Theophylline already has a narrow safety window, so even modest bumps matter. Watch for a racing or pounding heart, jitteriness, trouble sleeping, nausea, or feeling shaky, which can be made worse by the ephedrine in your product too. A cup or two of tea may be fine, but please check with your pharmacist or doctor before relying on it daily.
