Here is the interaction we found between these two, with the evidence behind it.
ESTROGENS
Why this applies to Tibolone
Tibolone falls under ESTROGENS — the group Black Tea interacts with.
Black tea contains caffeine. Estrogen inhibits caffeine metabolism.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the key thing to know: this interaction isn't really about Tibolone getting stronger or weaker. It works the other way around. Tibolone acts like estrogen in the body, and estrogen can slow down how fast your body clears caffeine. So the caffeine from your black tea may hang around longer than usual.
In plain terms, the same couple of cups you always drink might hit a little harder. You could notice things like feeling jittery, a faster or pounding heartbeat, trouble sleeping, or feeling restless. This is considered a moderate, likely interaction, and it's easy to manage. If you feel wired, just cutting back on tea or coffee usually helps. Chat with your pharmacist or doctor if it keeps bothering you.

From our pharmacist’s overview on the Tibolone page
“Grapefruit is the risk I would learn first with tibolone (Livial), a hormone therapy used for menopause symptoms. Grapefruit can raise estrogen levels in the blood, magnifying both the drug's effects and its side effects, and it shares major status with activated charcoal, which soaks up medications in the gut and can blunt tibolone's…”

About Black Tea
What is Black Tea?
- Mental alertness and energy
- Heart health support
- Blood pressure and cholesterol
- Digestive complaints
- Weight management
- Antioxidant support
Black tea is a popular caffeinated drink made from the fully oxidized leaves of the Camellia sinensis plant, and it contains caffeine and antioxidant plant compounds. Moderate tea drinking is generally safe for most adults and may modestly support heart health and alertness, but it is not a proven treatment for any disease. If you are pregnant, breastfeeding, sensitive to caffeine, or taking medicines, talk to your pharmacist or doctor.
