Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Borage interacts with.

Theoretically, borage seed oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In healthy individuals, borage seed oil supplementation does not seem to affect platelet aggregation. However, gamma-linolenic acid, a constituent of borage seed oil, seems to decrease platelet aggregation by 45% and increase the risk of bleeding by 40% in animal and clinical research.

Here's the thing about alteplase (Activase): it's a powerful clot-busting drug given in the hospital for emergencies like a stroke or heart attack. It works fast to break up clots, and the biggest concern with it is bleeding. Borage seed oil contains GLA, which may make your platelets a little 'stickier-resistant' and slightly raise bleeding risk on its own.

Put the two together and, in theory, you could nudge that bleeding risk up a bit further. In real life this matters less than it sounds, since tPA is only used in a controlled hospital setting where doctors are watching you closely. The evidence here is fairly weak and mostly theoretical. Still, always tell the care team about any supplements like borage you take.
