Nafcillin Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the thing to know about pairing borage with nafcillin. This isn't about your antibiotic working better or worse. Nafcillin is an antibiotic that fights staph infections, and it happens to speed up a liver enzyme called CYP3A4. That same enzyme is what turns the natural plant toxins in some borage products, called pyrrolizidine alkaloids (PAs), into more harmful, liver-damaging byproducts.

So the worry is theoretical and centers on your liver, not on your infection. The good news: borage seed oil usually has little to no PAs, and quality products are labeled 'PA-free.' Borage leaf, flower, or whole-seed preparations are the riskier ones. To be safe, only use a certified PA-free product, and check with your pharmacist or doctor first.
