Cabotegravir, Rilpivirine falls under GLUCURONIDATED DRUGS — the group Calcium D-glucarate interacts with.
The calcium D-glucarate metabolite, D-glucaro-1,4-lactone, inhibits the enzyme beta-glucuronidase, reducing deconjugation of glucuronides in the intestine and thereby reducing reabsorption of the drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something worth knowing if you're getting your Cabenuva injections (cabotegravir plus rilpivirine). Cabotegravir is partly cleared by the body through a process called glucuronidation, where the liver tags the drug so it can leave in bile and stool. Calcium D-glucarate is thought to block an enzyme (beta-glucuronidase) that would normally recycle some of that drug back into the body. In theory, that could mean a bit more of the medicine gets flushed out, lowering its levels.
Now, the honest part: this is mostly theoretical, based on lab thinking rather than real patient studies. But with HIV therapy, even small drops in drug levels matter, since the goal is keeping the virus fully suppressed. I'd just run it by your HIV provider or pharmacist before adding this supplement.
