Here's the simple version. Fentanyl is a very strong opioid pain medicine, and one of its main effects is calming or slowing down the brain and breathing. The dream herb (Calea zacatechichi) seems to have its own mild calming effect on the nervous system, at least in animals and a few human reports. So in theory, layering the two together could deepen that sedation more than fentanyl alone would.

What that might look like in real life: extra drowsiness, feeling foggy or dizzy, and in a worst case, slowed breathing. I want to be honest though, this is a theoretical concern based on how these things work, not from solid studies. It's still worth telling your doctor or pharmacist before you mix them, since fentanyl is powerful and leaves little room for error.
