Why this applies to Tocilizumab
Tocilizumab falls under HEPATOTOXIC DRUGS — the group Camphor interacts with.
There have been a few case reports of transient elevations in liver enzymes in adults after oral or topical use of camphor. There has also been one case of hepatotoxicity in an infant after topical camphor use.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Tocilizumab (Actemra) and camphor. Tocilizumab can sometimes raise your liver enzymes on its own, which is why your doctor checks your liver bloodwork while you're on it. Camphor has been linked, in a small number of case reports, to temporary bumps in liver enzymes too. So the concern is simply that stacking two things that can stress the liver might add up.
I want to be honest about the strength of this one: it's mostly theoretical, based on how these substances behave, not on solid proof. Camphor used the right way (rubbed on the skin in small amounts) is far less worrisome, and it should never be swallowed. Tocilizumab isn't a prodrug, so camphor doesn't change how well it works. Just give your pharmacist or doctor a heads-up before using camphor products, and keep up with your liver monitoring.

Camphor is a strong-smelling compound from the camphor tree (or made synthetically) that is used in many topical rub-on products for muscle aches, itching, and chest congestion. Tocilizumab products block IL-6, a protein that drives inflammation. The label for Actemra lists several uses in adults and children.
Actemra is used for rheumatoid arthritis, giant cell arteritis, systemic sclerosis-associated interstitial lung disease, polyarticular and systemic juvenile idiopathic arthritis, CAR T cell-induced cytokine release syndrome, and COVID-19 in hospitalized patients.
