CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Acetaminophen, Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's what matters with this combo: the part that interacts is the caffeine in your Excedrin. Caffeine is cleared by a liver enzyme called CYP1A2, and CBD can slow that enzyme down. So instead of breaking caffeine down at its usual pace, your body may hold onto it longer, letting it build up.
In real life that means the caffeine may hit harder than you expect. Think jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious or wired. This is backed by good clinical studies, though the bigger effects showed up at fairly high CBD doses. The acetaminophen and aspirin parts aren't the concern here. If you regularly use both, just check with your pharmacist or doctor so you can keep your caffeine load comfortable.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the good news on Excedrin (acetaminophen, aspirin, and caffeine): even though CBD can slow down the CYP3A4 enzyme and raise levels of certain medicines, none of these three ingredients lean heavily on CYP3A4 to be cleared. Acetaminophen and aspirin are mostly handled by other pathways, and caffeine is broken down mainly by a different enzyme (CYP1A2). So this particular combo isn't a strong match for the interaction on file.
That said, CBD can mildly affect liver enzymes in general, and caffeine sensitivity varies person to person. If you notice extra jitteriness or sleeplessness, or you use Excedrin often, it's worth a quick chat with your pharmacist or doctor to be safe.

GLUCURONIDATED DRUGS

In vitro research shows that cannabidiol inhibits uridine diphosphoglucuronosyl transferase (UGT) 1A9 and UGT2B7, enzymes responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Excedrin (which combines acetaminophen, aspirin, and caffeine) and CBD. Your liver clears acetaminophen partly through a process called glucuronidation, and lab studies suggest CBD might slow down some of the enzymes (UGT1A9 and UGT2B7) that handle this step. In theory, that could let a bit more acetaminophen linger in your system, which matters because acetaminophen can stress the liver at high amounts.
But I want to keep this in perspective. This is based only on test-tube research, not real people, so we honestly don't know how big a deal it is day to day. Still, since acetaminophen is hard on the liver in excess, I'd avoid combining CBD with regular Excedrin use without checking first. Please run it by your pharmacist or doctor before pairing them.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that cannabidiol inhibits CYP2E1. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters with Excedrin and CBD: the acetaminophen part of that combo is partly handled by a liver enzyme called CYP2E1. In a test-tube setting, CBD has been shown to slow that enzyme down. If that happened in real life, acetaminophen could clear a little more slowly and build up slightly. With acetaminophen, the main worry from higher levels is stress on the liver, so it's worth being mindful, especially if you already take other products containing acetaminophen.
That said, this is purely a theoretical concern from lab work, and it has not actually been reported in people. The aspirin and caffeine pieces aren't the issue here. Still, it's smart to mention your CBD use to your pharmacist or doctor so they can keep your total acetaminophen in a safe range.

CAFFEINE

Caffeine is a substrate of CYP1A2, and cannabidiol has been shown to inhibit CYP1A2 metabolism. A pharmacokinetic study in healthy adults shows that taking oral cannabidiol, starting at 250 mg once daily and titrating to 750 twice daily over a total of 24 days, increases the peak serum level of caffeine by 15% and the overall exposure to caffeine by 95% after a single dose of caffeine 200 mg taken on day 23. Other clinical research also shows that cannabidiol modestly increases the area under the curve of caffeine but does not increase peak serum levels of caffeine.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the piece to pay attention to with Excedrin: it contains caffeine, and CBD can slow down how your body clears caffeine. CBD blocks a liver enzyme called CYP1A2 that normally breaks caffeine down, so the caffeine hangs around longer. In a good-quality study, regular CBD nearly doubled overall caffeine exposure from a single dose.
What might you notice? More of the typical caffeine effects: feeling jittery or wired, a racing heart, trouble sleeping, or an upset stomach. The aspirin and acetaminophen parts of Excedrin aren't really the concern here. This usually matters more with higher, daily CBD doses than a tiny one-time amount. If you use both, just mention it to your pharmacist or doctor before changing anything.
