This combination cold-and-cough product already has two ingredients that can make you drowsy: hydrocodone (an opioid painkiller and cough suppressant) and chlorpheniramine (a sedating antihistamine). CBD, especially at higher doses, can add its own calming, sleepy effect on top of those. So the main concern here is that the two could pile up and leave you more drowsy, foggy, dizzy, or slowed down than expected.

I'll be honest, this is a theoretical concern based on how these things work, not proven in solid studies, so don't panic. Still, with an opioid in the mix it's worth respecting. If you do combine them, go slow, avoid driving until you know how you feel, and check with your pharmacist or doctor first.

In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

This combination cold and pain product contains hydrocodone, an opioid that your body partly turns into a more active painkiller using a liver enzyme called CYP2D6. CBD may, in theory, slow that enzyme down. Because hydrocodone is partly a prodrug, blocking CYP2D6 could actually mean a bit less pain relief rather than more, though the effects here are not fully predictable.

Honestly, this is based on lab studies, not real-world cases, and one human trial found CBD did not block this enzyme much. So I wouldn't lose sleep over it. Still, if you notice extra drowsiness, dizziness, or that your pain isn't well controlled, check in with your pharmacist or doctor before changing anything.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

This combo product has a few moving parts, but the one that really matters here is the caffeine. CBD can slow down a liver enzyme called CYP1A2, which is the main way your body clears caffeine. When that pathway is blocked, caffeine hangs around longer and can build up. In practice, that might mean you feel more jittery, your heart races a bit, you have trouble sleeping, or you notice an upset stomach, basically too much caffeine even from a normal dose.

The good news is this is usually manageable. The studies that showed it used fairly high CBD doses, and the effect is moderate, not dangerous. Still, if you take CBD regularly, it's worth a quick chat with your pharmacist or doctor before pairing it with this medicine.

This combination product contains hydrocodone, an opioid pain reliever that your liver processes partly through the CYP3A4 enzyme. CBD can slow that enzyme down. In plain terms, the hydrocodone may clear out of your body a little more slowly and build up higher than usual. That can mean stronger opioid effects, like extra drowsiness, dizziness, constipation, or slowed breathing if levels climb too high.

The chlorpheniramine (an antihistamine) can add to that sleepiness too. None of this means you can't ever use them together, but the increased sedation is worth taking seriously. Please check with your pharmacist or doctor before combining CBD with this medicine, and don't change anything on your own.

This is a combination product, and a couple of its ingredients (especially acetaminophen) are cleared in part through a process called glucuronidation. CBD has been shown in lab dish studies to slow down some of the enzymes (UGT1A9 and UGT2B7) that handle that step. In theory, that could let levels of these ingredients linger a bit higher than usual.

If that happened, you might notice more of the expected effects, like extra drowsiness or grogginess from the hydrocodone and chlorpheniramine. Keep in mind this is only test-tube evidence, not proven in people, so it may not amount to much in real life. Still, since acetaminophen overload can stress the liver, I'd run this combo past your pharmacist or doctor before pairing it with CBD.

This combo product (Hycomine Compound) contains a few ingredients, but the one tied to this interaction is acetaminophen (Tylenol). A small piece of how your body handles acetaminophen runs through an enzyme called CYP2E1. In lab dishes, CBD seems to block that enzyme. The thinking is that CBD could slow this pathway and let a bit more acetaminophen hang around.

Here's the honest part: this has only been seen in test tubes, never confirmed in real people, so I wouldn't lose sleep over it. Still, since acetaminophen is hard on the liver in large amounts, it's worth being mindful not to overdo total daily acetaminophen, especially with regular CBD use. The hydrocodone, chlorpheniramine, caffeine, and phenylephrine aren't the concern here. Check with your pharmacist or doctor if you use both regularly.

Caffeine is a substrate of CYP1A2, and cannabidiol has been shown to inhibit CYP1A2 metabolism. A pharmacokinetic study in healthy adults shows that taking oral cannabidiol, starting at 250 mg once daily and titrating to 750 twice daily over a total of 24 days, increases the peak serum level of caffeine by 15% and the overall exposure to caffeine by 95% after a single dose of caffeine 200 mg taken on day 23. Other clinical research also shows that cannabidiol modestly increases the area under the curve of caffeine but does not increase peak serum levels of caffeine.

The piece that matters here is the caffeine. Your liver clears caffeine using an enzyme called CYP1A2, and CBD can slow that enzyme down. In a good-quality study, regular CBD nearly doubled the body's overall exposure to a single caffeine dose.

In plain terms, the caffeine in this product may stick around longer and feel stronger. You might notice more jitteriness, a racing or pounding heart, trouble sleeping, or feeling on edge.
