Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the practical concern with mixing CBD and Eliquis (apixaban). Your liver uses an enzyme called CYP3A4 to help break down apixaban and clear it from your body. CBD can slow that enzyme down, which may let more apixaban build up than usual. Since apixaban is a blood thinner, higher levels can mean a greater chance of bleeding, things like easy bruising, nosebleeds, bleeding gums, or in more serious cases blood in the urine or stool.

Apixaban is not a prodrug, so this goes in the direction you'd expect: less breakdown means a stronger effect. The evidence here is graded as solid, so I'd take it seriously. Please don't stop or change anything on your own. Talk with your pharmacist or doctor first so they can guide you safely.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
