Aspirin, Hydrocodone falls under CNS DEPRESSANTS — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might have additive effects if used with other CNS depressants.
Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

The combination you're asking about pairs aspirin with hydrocodone, an opioid pain reliever. Hydrocodone already slows down the brain and body, which is why it can make you feel drowsy, foggy, or short of breath at higher doses. CBD, especially in higher supplement doses, may add a calming, sedating effect of its own. Put them together and those effects can stack up, so you might feel sleepier, more lightheaded, or less alert than you'd expect.

To be fair, this concern is mostly theoretical, based on how CBD behaves rather than strong human studies. Still, it's worth being careful, especially before driving. If you'd like to use CBD while taking this medicine, please run it by your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.

Here's the wrinkle with this combo: the hydrocodone in your tablet is a prodrug. Your body uses an enzyme called CYP2D6 to turn part of it into hydromorphone, which is a stronger painkiller. Lab tests suggest CBD can slow CYP2D6 down. In theory, that could mean a little less of that conversion, so you might notice your pain isn't covered quite as well. (The aspirin part isn't really affected here.)

The good news: this is only a theoretical concern based on test-tube data, and a human study using a similar CYP2D6 drug showed CBD didn't actually block it. So real-world impact is likely small. Still, since CBD products vary so much, it's worth a quick chat with your pharmacist or doctor before pairing them.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

This combination product pairs aspirin with hydrocodone, an opioid painkiller. The piece that matters here is the hydrocodone, which your liver partly handles using an enzyme called CYP3A4. CBD can slow that enzyme down, so hydrocodone may clear a little more slowly and build up in your system. That can mean stronger opioid effects than you expect, such as extra drowsiness, dizziness, slowed breathing, constipation, or feeling foggy.

The evidence that CBD blocks CYP3A4 is solid, though it has been studied most with other medicines, not specifically this one. The aspirin part isn't really affected this way. To be safe, don't start or stop anything on your own. Have a quick chat with your pharmacist or doctor first, especially since concentrated CBD supplements act differently than tiny food amounts.
