Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for Vraylar (cariprazine): your liver uses an enzyme called CYP3A4 to clear it out. CBD can slow that enzyme down, which means cariprazine may stick around in your body longer and build up to higher levels than usual. Cariprazine isn't a prodrug, so more of it in your blood simply means a stronger effect.

In practice, that could mean more of cariprazine's typical side effects, like drowsiness, restlessness or an urge to keep moving (akathisia), tremor, dizziness, or nausea. The lab evidence here is solid, and CBD has clearly raised levels of other CYP3A4 drugs in people. Don't change anything on your own. Just check with your pharmacist or doctor before combining them so they can watch how you feel.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and teens 13 and older, and to treat acute manic or mixed episodes associated with bipolar I disorder in adults and children 10 and older. In adults, it is also approved for depressive episodes associated with bipolar I disorder (bipolar depression).

Additionally, Vraylar is approved as an add-on therapy alongside antidepressants for adults with major depressive disorder (MDD) who need extra help beyond their current antidepressant alone.
