Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's something worth knowing if you're pairing CBD with dronabinol. Your body uses an enzyme called CYP2C9 to help break down dronabinol, and CBD can slow that enzyme down. When the cleanup slows, more dronabinol can linger in your system than expected. Dronabinol is itself a cannabinoid (it's basically THC), so it's active as-is, not a prodrug that needs switching on.

In plain terms, the two together may push dronabinol's effects up: more drowsiness, dizziness, a 'high' or foggy feeling, fast heartbeat, or low blood pressure when you stand. This is graded as a probable, moderate interaction with decent evidence behind the enzyme effect. Please don't change anything on your own. Just check with your pharmacist or doctor so they can watch your dose and how you feel.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the simple version: your body uses a liver enzyme called CYP3A4 to help break down dronabinol. CBD can slow that enzyme down, so dronabinol may clear out of your system more slowly and build up higher than expected. This isn't a prodrug situation, so more dronabinol in your blood means a stronger effect, not a weaker one.

What might that look like? More drowsiness, dizziness, a racing heart, confusion, or feeling unusually "high" or anxious. The evidence here is solid for CYP3A4 in general, though it hasn't been studied with dronabinol specifically. Please don't start, stop, or adjust anything on your own. Just loop in your pharmacist or doctor first, since they may want to watch your dose more closely.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
