Here's the gist for this pairing. Erythromycin is a well-known blocker of the liver enzyme CYP3A4, and your body uses that same enzyme to help break down CBD. So when you take them together, erythromycin can slow the cleanup of CBD, letting it build up a bit more than usual. In plain terms, you may feel CBD's effects (and any side effects) more strongly, things like drowsiness, lightheadedness, dry mouth, diarrhea, or changes in appetite.

I want to be honest about how solid this is: it's a theoretical concern based on how these two work, not something proven in studies, so it may not amount to much for everyone. Still, since CBD product strength varies so much, it's worth a quick chat with your pharmacist or doctor before combining them.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for this pairing. Erythromycin is cleared in part by a liver enzyme called CYP3A4, and CBD can slow that enzyme down. When the cleanup crew works slower, more erythromycin can hang around in your blood than expected. It's not a prodrug, so this points toward a stronger erythromycin effect, not a weaker one.

What that can look like with erythromycin is more stomach upset, nausea, cramping, or diarrhea. A bigger concern is that erythromycin can affect heart rhythm (a QT prolongation effect), and higher levels may add to that risk. Please don't stop or change anything on your own. Just loop in your pharmacist or doctor so they can check your other meds and decide what's safe.
