Lesinurad falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the gist: your liver uses an enzyme called CYP2C9 to help clear lesinurad (Zurampic) from your body. CBD can slow that enzyme down. When that happens, lesinurad isn't broken down as quickly, so its level in your blood can creep up. Lesinurad is the active drug itself (not a prodrug), so more of it sticking around means a stronger effect than intended.

In real terms, that could mean more strain on your kidneys, since lesinurad already carries a warning about kidney problems, plus possible headache or reflux. The evidence here comes from solid human studies with similar CYP2C9 drugs, so it's worth taking seriously. Please don't stop or change anything on your own. Check with your pharmacist or doctor first, and keep an eye on your kidney function.

Here's the thing about pairing CBD with lesinurad (Zurampic): it's actually the medication that may affect the CBD, not the other way around. Lesinurad has a mild ability to speed up a liver enzyme called CYP3A4, and that same enzyme helps break down CBD. So in theory, taking them together could push your CBD levels a little lower and make your CBD feel less effective.

I want to be honest with you, though: this is a theoretical concern based on how these compounds behave in the body, not something proven in studies. Your lesinurad should keep doing its job for your gout and uric acid. If you use CBD and feel it's not working as well, just chat with your pharmacist or doctor before changing anything.

What Is Lesinurad Used For?

Brand name: Zurampic

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
