Here's the simple version: both CBD and perampanel (Fycompa) can make you drowsy on their own. When you put them together, that sleepy, foggy feeling can stack up. Perampanel is already known for causing dizziness, sleepiness, trouble with balance, and sometimes irritability or mood changes. Adding CBD, especially at higher supplement doses, could make those effects feel stronger.

I'll be honest with you, this is a theoretical concern based on how these two work, not proof from large studies. So it's not a reason to panic. But because perampanel already affects how alert and steady you feel, it's worth a quick chat with your pharmacist or doctor before combining them, and please be careful with driving until you know how you react.

Cannabidiol may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for these two together: CBD can slow down one of the liver pathways (called CYP2C19) that helps clear perampanel from your body. Perampanel works as-is, it isn't a prodrug that needs activating, so when CBD slows its breakdown, more of the drug can build up. That means you might feel its effects more strongly than usual.

With perampanel, that could show up as extra drowsiness, dizziness, trouble walking steadily, blurred vision, or mood and irritability changes. We know CBD reliably blocks this enzyme (solid research backs it), so this isn't just theory. Please don't change anything on your own. Loop in your pharmacist or doctor first, since your seizure dosing may need a closer look.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist for Perampanel (Fycompa): your liver uses an enzyme called CYP1A2 to help clear it out. CBD can slow that enzyme down a bit, so perampanel may not break down as quickly. The likely result is a little more perampanel hanging around in your system than usual.

Practically, that could mean stronger side effects, like extra drowsiness, dizziness, trouble with balance, or mood and irritability changes that perampanel is already known for. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
