Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to clear rosiglitazone (Avandia). CBD can slow that enzyme down, so the medication may hang around longer and build up a bit in your system. Rosiglitazone is an active drug (not a prodrug), so more of it means a stronger effect than expected.

In practice, that could mean your blood sugar runs lower than usual, or you notice more of its known effects like fluid retention, swelling in the legs, or weight gain. This is graded as moderate and probable, and good human studies back the enzyme effect, though most data come from other CYP2C9 drugs. Please don't change anything on your own. Check with your pharmacist or doctor first, and keep an eye on your blood sugar.

Here's the situation with CBD and your rosiglitazone (Avandia). Your liver uses an enzyme called CYP2C8 to clear this diabetes medicine out of your body. Lab studies suggest CBD can slow that enzyme down. If that happens, rosiglitazone could build up a bit higher than usual, which might mean more of its typical side effects, things like fluid retention and swelling in the legs, weight gain, or low blood sugar (shaky, sweaty, lightheaded feelings) if you also take other diabetes medicines.

I want to be honest though: this is based only on test-tube research and hasn't actually been seen in real patients. So it's a theoretical concern, not a proven problem. Still, it's worth a quick chat with your pharmacist or doctor before adding CBD, especially since product strengths vary so much.

What Is Rosiglitazone Used For?

  • Rosiglitazone is used along with a diet and exercise program and sometimes with one or more other medications to treat type 2 diabetes (condition in which the body does not use insulin normally and therefore cannot control the amount of sugar in the blood). Rosiglitazone is in a class of medications called thiazolidinediones. It works by increasing the body's sensitivity to insulin, a natural substance that helps control blood sugar levels. Rosiglitazone is not used to treat type 1 diabetes (condition in which the body does not produce insulin and therefore cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
