Why this applies to Ruxolitinib
Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear ruxolitinib (Jakafi, Opzelura). CBD can slow that enzyme down, so the drug may not get cleared as quickly and can build up to higher levels in your blood. Ruxolitinib works as-is (it is not a prodrug), so higher levels generally mean a stronger effect and a higher chance of side effects.
With ruxolitinib, that could show up as lower blood counts (more bruising, bleeding, fatigue, or infections), dizziness, or headache. The evidence that CBD blocks this enzyme is solid, so this is worth taking seriously. Please don't stop or adjust either one on your own. Loop in your oncologist or pharmacist first, since they may want to monitor your blood counts or rethink the CBD.

What Is Ruxolitinib Used For?
Brand names: Jakafi, Opzelura
Ruxolitinib is used in two very different ways. Oral products (Jakafi and Jakafi XR) treat certain blood and bone marrow conditions and graft-versus-host disease. A topical cream (Opzelura) treats certain skin conditions.
Each use belongs to a specific product. Jakafi/Jakafi XR are not skin treatments, and Opzelura is not approved for the blood conditions.
