Sotorasib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist for Lumakras (sotorasib): your liver uses an enzyme called CYP3A4 to help clear this cancer medicine from your body. CBD can slow that same enzyme down. When CYP3A4 is held back, sotorasib can clear more slowly and build up to higher levels than expected.

In real life, higher sotorasib levels could mean more of its usual side effects, things like nausea, diarrhea, fatigue, or changes in your liver blood tests. The evidence that CBD blocks CYP3A4 is solid, though it hasn't been studied with sotorasib specifically. Sotorasib is an active drug, not a prodrug, so this goes in the direction of more effect, not less. Please loop in your oncology team or pharmacist before mixing the two.

Cannabidiol is a substrate of CYP3A4 enzymes. Theoretically, drugs that induce CYP3A4 enzymes might reduce the levels and effects of cannabidiol.

Here's the key thing to notice: in this interaction, it's your CBD that may take the hit, not the sotorasib. Sotorasib can speed up a liver enzyme called CYP3A4, and CBD is broken down by that same enzyme. So if sotorasib revs it up, your body may clear the CBD faster, leaving lower levels and possibly less of whatever benefit you're hoping to get from it.

I want to be honest that this is theoretical, based on how these compounds behave in the lab, not on real patient reports, so it may not amount to much in practice. Still, since you're being treated for cancer, please loop in your oncology team or pharmacist before adding CBD. They can make sure nothing affects your sotorasib treatment.

About Sotorasib

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
