Sulfamethoxazole, Trimethoprim falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP2C9.
In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's the gist with this pairing. The sulfamethoxazole part of Bactrim is partly cleared by a liver enzyme called CYP2C9. CBD can slow that enzyme down, so in theory the antibiotic may hang around a bit longer and reach higher levels in your blood. That could nudge up the chance of typical Bactrim side effects, things like nausea, an upset stomach, skin rash, or sun sensitivity.

That said, this enzyme effect has been clearly shown for a few other drugs (like losartan and topiramate), not specifically for Bactrim, so the real-world impact here is more of an educated guess. Bactrim is usually a short course, which helps. Still, it's worth a quick word with your pharmacist or doctor before combining them, especially with concentrated CBD products.
