Telmisartan falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Cannabidiol (cbd) interacts with.

Theoretically, CYP2C19 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP2C19 enzymes. Theoretically, drugs that inhibit CYP2C19 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the key thing to know: in this pairing, it's the CBD that may be affected, not your telmisartan. Telmisartan isn't a prodrug and doesn't rely on the CYP2C19 enzyme to work. But if telmisartan acts as a mild blocker of that enzyme, it could in theory slow how your body breaks down CBD, letting CBD levels creep up a bit.

If that happens, you might notice more of CBD's own effects, like drowsiness, tiredness, dry mouth, or an upset stomach. This is only a theoretical concern based on how the enzyme works, not real-world reports, so I wouldn't lose sleep over it. Still, it's worth a quick chat with your pharmacist before adding a CBD product, especially a concentrated one.

What Is Telmisartan Used For?

Brand name: Micardis

Telmisartan tablets — sold as Micardis and the generic telmisartan — treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of strokes and heart attacks.

Micardis and telmisartan tablets are also approved to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients 55 or older who are at high cardiovascular risk and cannot take ACE inhibitors. This use is for patients with a history of conditions such as coronary artery disease, peripheral arterial disease, stroke, TIA (mini-stroke), or high-risk diabetes with signs of organ damage.
