Why this applies to Tretinoin

Tretinoin falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2C8.
In vitro research shows that cannabidiol inhibits CYP2C8. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2C8 substrates might increase the risk for adverse effects from these substrates.

Here's the gist: when we're talking about oral tretinoin (the capsule form, Vesanoid, used for the leukemia called APL), your liver helps clear it using an enzyme called CYP2C8. Lab studies suggest CBD can slow that enzyme down. In theory, that could let tretinoin build up a bit and make its side effects more noticeable, things like headache, dry skin and lips, bone pain, or the dryness and dizziness people often feel on this drug.

I want to be honest about the strength here: this is based only on test-tube research and hasn't actually been seen in real patients, so it's more of a caution than a proven problem. It also doesn't apply to the skin creams and gels (Retin-A, Renova, Altreno), since very little gets into your bloodstream. If you're on the oral form, please loop in your oncology team before adding CBD.

Tretinoin cream 0.02% is approved to help lessen the look of fine facial wrinkles in people who also follow a comprehensive skin care and sun-avoidance program — including daily sunscreen, protective clothing, and moisturizers.

It's important to know what tretinoin does not do: it will not erase wrinkles, reverse sun damage, repair deeply weathered skin, or restore younger-looking skin. It has also not been shown to help with coarse or deep wrinkles, rough skin texture, dark spots, or sagging skin.
