Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for Cabenuva (cabotegravir plus rilpivirine). Of the two, rilpivirine is partly cleared by an enzyme called CYP3A4. In lab dish studies, compounds in cannabis can slow that enzyme down a little, which in theory could let rilpivirine levels creep up. If that happened, you might notice more of its usual effects, like headache, trouble sleeping, odd dreams, or mood changes.

I want to be honest with you though: this is only test-tube data, not real people, so it's a weak and theoretical concern. Cabenuva is a long-acting injection your provider gives you, and staying on track with HIV treatment matters a lot. Don't change anything on your own. Just mention your cannabis use to your doctor or pharmacist so they can keep an eye on things.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
